The Financial Aid Office uses a number of forms in gathering information for each student's financial aid file. One or more of these forms may be needed to complete your financial aid file. For your convenience we have listed various forms below, if one is needed, feel free to print the form, complete the information, and fax it to the Financial Aid Office at: 517.552.8022.
Financial Aid Consent Form
Gives the Financial Aid Office the authority to release a student's financial aid information to an immediate family member.
Financial Aid Data Form
This form is to gather general information for a student's financial aid file.
Independent Student Verification Worksheet
To verify untaxed income and other items that may not be listed on an income tax return such as child support, number of family members in household, names, and schools where other family members may be attending other colleges or universities, etc.
Professional Judgment Policy & Forms
For students who have already applied for financial aid and have found that their circumstances have changed significantly (i.e. loss of employment, etc).
Professional judgment decisions are made on a case-by-case basis. Requests must be in writing and appropriate forms and third party documentation must be submitted for consideration. Contact the Financial Aid Office for more assistance.
